FUTURE CANCELLATION DECLARATION
Identify the subscription and requested date.
When enabled, the form must validate the contract or customer reference, the desired termination date and any extraordinary reason separately. Ordinary cancellation stops future renewal at the applicable date; it does not itself request withdrawal or a refund.
Future receipt contract: after a real server-side write succeeds, the page must show a durable receipt ID, UTC timestamp, exact declaration copy, processing status and a saveable copy. Email acknowledgment must be reported separately and never claimed if email delivery fails.
